At Long Lake Sports & Orthopaedic Physiotherapy, we strive to eliminate any concerns you may have regarding the confidentiality of your care and the protection of your personal information.
The following points outline how our clinic collects, protects, and discloses the personal information you provide to us. Long Lake Sports & Orthopaedic Physiotherapy values your privacy and operates in compliance with the Personal Information Protection Act (PIPA).

We collect, use, and disclose personal information only for the purposes of providing physiotherapy or kinesiology services and the administration of those services.
Your written consent is required prior to disclosing your personal information to third parties. You can withdraw your consent to release information at any point in time (in writing).
All staff are educated and trained on the importance of protecting your personal information and are held accountable for implementing the safeguards and security measures designed to protect it, including:

Physical Safeguards
o Monitored alarm system
o Deadbolt entry lock
o Locked filing cabinets
o Private treatment rooms
o Private offices to conduct telephone conversations
o Secure location of fax machine

Technological Safeguards

o Password protected computer operating systems and software
o Encrypted email communication
o Virus scanning software

Administrative Safeguards

o Access to personal information is restricted to authorized users on a need to know basis.
o All staff have signed confidentiality agreements that extend beyond the term of their employment.
o Any services on site (cleaning, computer support) and off site (medical couriers) are held to strict confidentiality measures.

Your records will be securely stored for 15 years at which time all paper records are shredded and electronic records are securely disposed of.  You can request a copy of your records in writing at any point in time, and it will be provided to you in a timely manner for a reasonable fee.

Long Lake Physiotherapy is conveniently open from 7:00 am-7:00 pm Monday to Thursday and 7:00 am-5:00 pm on Friday.

 

What if I am unable to attend a scheduled appointment?2016-06-20T16:57:40+00:00

We require 24 hours notice to ensure you will not be charged our cancellation fee of $35.  Sufficient notice is greatly appreciated and enables our staff to assist other patients in seeing their physiotherapist in a timely manner.

What do I need to bring to my physiotherapy appointment?2016-06-20T16:57:40+00:00
    • Loose fitting clothing that adequately exposes the body area you wish to have examined.
    • Athletic shoewear.
    • For ICBC and WSBC claims only -> your MSP Health Care Card, claim number, and the contact information for your adjuster/claim manager.
    • Doctor’s referral if you have one (not required).
    • The name(s) of any medication you are taking.

No, a doctor’s referral is not required to see a physiotherapist in British Columbia.

Some extended health care plans may require a doctor’s referral.
